Don't need AI for this.  My health insurance refused to refund a blood test, despite it being one of the items explicitly named in my insurance policy, because the it was a "prise de sang" type of blood-test.  I spoke to two different humans about this on the phone... two humans who aren't fluent in French else they'd know what "prise de sang" means.

JFC.  If this happened recently, you might be able to call the provider and have them switch the coding.  Does your province have any kind of insurance regulatory board?  They might be able to do something.  If nothing else, the OQLF should be able to call them up and swear at them in French.  Or send in their elite combat forces.
